How it works
The app follows a clear case flow. Allowances and available report content depend on the selected tier.
1. Submit case evidence
Provide documents, images and a description for a genuine failure investigation, within your tier's approved file, size, format and page or image limits.
2. Answer clarifications
The AI asks clarification questions generated from your own submitted case evidence. The available clarification rounds depend on your tier.
3. Review AI results
The app produces tier-controlled results and a report. Basic, Standard and Expert use the same best-available FI engine; Expert additionally includes integrated RCA tools.
4. Add professional review
If needed, purchase a separate professional service. Qualified MTIS staff prepare a distinct, edited deliverable rather than re-labelling the AI-generated report.

Failure Investigator is for real components, products or systems that you or your organisation are currently investigating. It is not a general-purpose document-analysis or research tool.
The following are prohibited on every tier: fictitious cases, research-paper examples, bulk company-history uploads, and deliberate API-token or clarification-round consumption without a genuine investigative purpose.
