What happens after you send this

MTIS issues an invoice for the plan and term you select, to the billing details you give below.
The account is created for the named user, but stays inactive.
You pay the invoice by the route set out on it.
MTIS confirms payment and activates the account. The 12-month term begins on activation — not when you send this form, and not when payment is submitted.

This website does not take card payments. Nothing is charged here and no payment details are asked for. Payment is confirmed manually by MTIS staff, and subscriptions do not renew automatically.

Number of named users

Each subscription covers one named user. Accounts are not shared between people.

If your organisation needs a PO number on the invoice, give it here — it is far harder to add afterwards.

Tell us what you intend to use it for, the start date you need, and anything your finance team requires on the invoice. Twenty characters minimum.

Confirmation

Your request will be encrypted and stored outside the public website; the project mailbox receives only a random reference and receipt timestamp. See the Privacy Notice.

Sending this form does not create a subscription and does not charge you. It asks MTIS to issue an invoice. No card or bank details are collected on this website.
