Request a forensic investigation

A forensic investigation is a materials investigation run so that its findings can be relied on by someone other than you — an insurer, a supplier, a tribunal or a court.

Is this the right form? If nobody is disputing anything and you simply need to know why a component failed, use the project quote request instead — it is shorter. Use this form where liability, a claim or a dispute is in play, or may become so.

We cannot begin without a conflict check, so the parties involved are asked for first. Do not send privileged material through this form, and do not attach documents — it does not accept files. Once we have run the conflict check we will reply with a secure route for anything sensitive.

Names only — no case detail is needed here. Include the other side, any insurer, and any expert already instructed. MTIS cannot accept an instruction without this.

What failed, what is disputed about why, and what the parties disagree on. Do not include privileged material or personal data.

The evidence

What survives decides what can be established. Answer honestly — bounded answers from imperfect evidence are still useful, but only if we know what we are working with.

Where it has been, who has had access, and whether any record was kept. Chain of custody decides whether evidence is relied on or challenged.
